Does a no-shave hair transplant limit the number of grafts? LH-FUE vs “unshaven DHI”

No, a no-shave hair transplant does not by nature limit the number of grafts: what caps sessions is the protocol chosen. The most widespread “unshaven DHI” offers are often limited to around 800 to 1,500 grafts per session, because only a small window of the donor area is worked. LH-FUE (Long Hair FUE), on the other hand, is a complete long-hair extraction technique: it allows large sessions when the donor area permits — at the cost of a slower and more demanding procedure for the surgeon. Confusing these two approaches is the main source of patient disappointment.

Where does the idea that “no shave = few grafts” come from?

It comes from a commercial shortcut. In most clinics, “no-shave hair transplant” actually refers to a partially unshaven FUE or DHI: discreet strips are shaved and camouflaged by the long hair, or extraction is confined to a reduced window. This organisation mechanically limits the number of accessible follicles — hence the advertised ceilings of 800 to 1,500 grafts. Many websites conclude that no-shave transplantation is a “false good idea” reserved for small areas. That conclusion describes their protocols, not the LH-FUE technique.

What changes with true LH-FUE?

LH-FUE extracts the grafts with their long hair shaft, follicle by follicle, without shaving either the donor or the recipient area. The surgeon immediately sees the orientation and the look of the implanted hair, and the patient keeps his social appearance throughout recovery. Since extraction is not confined to a shaved window, the usable donor surface is the same as in classic FUE: the achievable number of grafts then depends on the real factors — density and laxity of the donor area, hair calibre, surface to be covered — and not on camouflage. The counterpart is real and deserves to be said: long-hair extraction is markedly slower and technically more demanding, which explains why few practitioners offer it and why an LH-FUE session requires more operating time than an equivalent shaved FUE.

How many grafts can be expected in one session?

There is no universal figure: the useful range is determined by examining the donor area, never from a photo or a catalogue. In FUE — shaved or long-hair — usual sessions range between 1,500 and 3,000 grafts depending on donor capital and indication; some extensive areas justify planning two surgical sessions rather than over-harvesting the donor area in one go. One principle never changes: the follicular capital is finite, and each harvested graft will not grow back in the donor area. Announcing a “maximum number of grafts” without an examination is not a sign of seriousness; preserving the capital for the future is.

How to check what you are really being offered?

Three questions are enough to clarify a “no-shave” offer. Who extracts the grafts — the surgeon himself, or other staff? Is extraction done with long hair over the whole donor area, or within a camouflaged shaved window? Does the quote state a number of grafts after examination of the donor area? At Dar El Hakim, LH-FUE extraction and implantation are performed by the surgeon at every step, and the number of grafts is set at the consultation, according to your donor capital and your goals — not to a flat fee.

Frequently asked questions

Does a no-shave hair transplant give fewer grafts than a classic FUE?

Not when it is a true LH-FUE: the usable donor surface is identical, so the graft potential is too. The ceilings of 800 to 1,500 grafts concern partial “unshaven” protocols that only use a limited window of the donor area.

Why do so few clinics offer LH-FUE?

Because long-hair extraction is slower and technically more demanding than a shaved FUE, and it lends itself poorly to high-volume organisation.

Is a large bald area compatible with LH-FUE?

Often yes, provided the donor area allows it. Depending on the surface to be covered, the surgeon may propose a large session or two surgical sessions to preserve the follicular capital. The precise answer belongs to the clinical examination.

Is the result different from a shaved FUE?

The biology of the graft is the same. LH-FUE adds two advantages: an immediate preview of the result (implanted long hair) and complete social discretion during recovery. Final density is assessed, as for any transplant, at 8–12 months.

How many grafts would I need?

It depends on the surface to be covered, your donor density and your hair calibre. Only an examination of the donor area allows a serious estimate; be wary of figures promised without an examination.
