LH-FUE: who is a good candidate? Hair loss stage, density, hair length
Updated: 17 August 2026 — Dr Khalil El Cadhi, ISHRS Full Member (Dar El Hakim, Djerba).
A good candidate for LH-FUE combines four elements: a donor area of adequate density, hair long enough to conceal the extraction sites (in practice 4 to 6 cm at least), hair loss that has stabilised enough for the plan to hold over time, and expectations consistent with what the technique delivers. The Norwood stage matters less than is usually written: what decides is the donor capital relative to the surface to be covered, not the stage number. The real constraint lies elsewhere — LH-FUE requires a markedly longer operating time, and the surgeon who performs it has to accept that pace.
Is the stage of hair loss the deciding criterion?
No, and this is the most widespread mistake. One frequently reads that no-shave hair transplantation is reserved for Norwood stages 2 to 3 and “not recommended” from stage 4 onwards: that statement describes “unshaven DHI” protocols with a limited donor window, not LH-FUE. A series published by Brigante and Wells in 2026 (Cureus, PMID 41755972) reports an average of about 4,600 follicular units per procedure in long-hair extraction, with a transection rate of around 3% — volumes that have nothing in common with a 1,500-graft ceiling. The useful criterion is therefore not the stage but the ratio between donor-area density and the surface to be covered. A Norwood 5 with a dense crown can be a better candidate than a Norwood 3 with a poor donor area — the question of the number of grafts in a no-shave hair transplant is covered in detail in our dedicated article.

How long does the hair need to be?
The hair must be long enough for the neighbouring hairs to fall back over the extraction sites, which in practice means 4 to 6 cm at least on the crown, more if the hair is fine or straight. This coverage is what makes LH-FUE socially discreet: without it, the technique loses its main point. A patient who habitually wears very short hair gains nothing from it and will be better served by a classic shaved FUE, which is faster and just as valid. A useful precision for reading offers: the classification published by Jimenez et al. in Dermatologic Surgery in March 2026 (PMID 41805884) defines five levels of “unshaven” transplantation and reserves the most complete level for long-hair extraction and implantation — shaving the recipient area does not enter into this count. Many commercial offers called “no-shave” fall within the intermediate levels.
What donor-area density is needed?
The assessment is made with a dermatoscope, by counting follicular units per square centimetre and appraising the number of hairs per unit, the hair calibre and the laxity of the scalp. Below a certain density, extracting with long hair becomes visible: the extraction zones no longer conceal themselves, which cancels out the intended benefit. Hair calibre counts as much as number: a thick hair covers more optically, a fine hair requires more grafts for a comparable result. Curly or frizzy hair conceals the extraction sites better but makes extraction technically harder, as the angle of the follicle is more difficult to follow — this is not a contraindication, it is a factor in operating time.
In which cases is LH-FUE not the right option?
It is not indicated when hair loss is still active and untreated in a young patient: transplanting onto an unstabilised process amounts to chasing after the baldness. Nor is it indicated in a patient who wants to be able to shave his head, when the donor area is too poor, or when a scalp condition (alopecia areata in an active phase, lichen planopilaris, uncontrolled inflammatory dermatosis) must be treated first. Finally, it makes no sense if the patient insists on a very large session concentrated into a short day: the long-hair procedure is intrinsically slower, as each graft must be extracted, preserved and implanted without its shaft being cut. This extra time is the price of discretion, and it must be stated up front.
Before the question of technique comes that of indication: transplant or treatment — five objective criteria to decide (in French).
How is candidacy assessed?
It rests on four measurements: the density and surface of the donor area, the recipient surface to be treated, hair length and calibre, and how long the hair loss has been going on together with any medical treatment. To this is added a conversation that counts as much as the figures: what the patient expects, which post-operative constraints he can accept (see recovery and social downtime after LH-FUE), and his availability for a long procedure. At the end of this assessment, three answers are possible — LH-FUE indicated, shaved FUE more relevant, or medical treatment first with reassessment later. The third answer is frequent and is not a refusal: it is a matter of sequence.
FAQ — candidacy for LH-FUE
Which stage of baldness allows a no-shave hair transplant?
There is no fixed threshold. The deciding criterion is the ratio between donor-area density and the surface to be covered, not the Norwood stage. The ceiling of 800 to 1,500 grafts often quoted describes “unshaven DHI” protocols with a limited window, not LH-FUE.
How long does the hair need to be for LH-FUE?
In practice 4 to 6 cm at least in the donor area, more if the hair is fine or straight. It is this length that conceals the extraction sites and gives the technique its whole point.
Can LH-FUE be done with curly hair?
Yes. Curly hair actually conceals the extraction zones particularly well. It does, however, make the procedure harder, as the angle of the follicle under the skin is more difficult to follow, which lengthens the operating time.
Is LH-FUE suitable for women?
It is often particularly well suited, as the length of the hair makes it possible to hide the after-effects entirely. The indication depends on the type of female alopecia involved, which must be diagnosed before any surgical project.
Should hair loss be treated before considering a transplant?
In progressive androgenetic alopecia, medical treatment is most often proposed before and after the procedure, to preserve the non-transplanted hair. A transplant redistributes a capital; it does not stop the miniaturisation process.
How long does an LH-FUE procedure take?
Markedly longer than a shaved FUE of equivalent volume, because of the handling of long-shaft grafts. The exact duration depends on the number of grafts and is specified after the assessment.

General information that does not replace a consultation. Hair transplantation is a surgical procedure reserved for the practitioner, subject to indication, contraindications and individual variability of results.
Not sure whether your donor area and hair length allow LH-FUE? A candidacy assessment measures donor density, the surface to be covered and the stability of your hair loss before proposing a technique.
